Cervical cancer develops through a multistep process that includes either three (CIN 1, CIN 2, and CIN 3), or, according to the Bethesda system, two lesion stages: low-grade squamous intraepithelial lesions and high-grade squamous intraepithelial lesions. The progression or regression of the lesions may depend, among other factors, on the host heritable immune response. Human leukocyte antigen-G (HLA-G) is a non-classical major histocompatibility complex class Ib molecule that acts as a specific immunosuppressor. In physiological conditions, HLA-G expression has a limited distribution, particularly occurring in cytotrophoblast cells, where it contributes to the fetal-maternal tolerance. On other hand, in the past few years, Th17 cells and their effector cytokines are increasingly being recognized as key players in inflammation, autoimmunity and allergic reactions. However, the nature of Th17 cells is elusively understood in cancer patients. In present study, we measured the levels of expression of Th17 and HLA-G and associated its levels in CIN 1 patients, chronic cervicitis (CC) patients and healthy controls (HC). In the group as a whole, HLA-G molecules and IL-17 were detected in 44 cases (75,9%) and 46 (79,31%) respectively. Among specimens that presented HLA-G expression, 29 out of 35 (82,8%) exhibited CIN I, 15 out of 23 (65,2%) exhibited CC. Similarly, considering only patients that presented IL-17 expression, 30 out of 35 (85,7%) patients exhibited CIN, 16 out of 23 (69,5%) exhibited CC and absence of HLA-G  and IL-17 expression were observed in HC group. HLA-G was significant (p-value = 0.04) with OR estimated at 6.61 (95% CI: 1.22, 49.55) indicating that the odds of having CIN 1 (compared to CC) is 6.61 times greater for women who express HLA-G than those who do not. Our data suggest that in CIN 1 patients the increased HLA-G could be associated with the progression of cervical lesion and presence of IL-17 may be a useful indicator for representing the severity of tissue injury suggesting   that Th17 cells are mediators during the immunologic process of CIN.
