SUMMARY:

Introduction: Chronic Low Back Pain (CLBP) is a global challenge, representing the primary cause of years lived with disability. Recent studies suggest that alterations in the Central Nervous System (CNS) play a crucial role in the chronification of pain and that treatment with Transcranial Direct Current Stimulation (tDCS) can modulate these brain areas, providing benefits for patients. Therefore, the combination of physical exercises, the gold standard for CLBP, and tDCS may offer synergistic benefits, promoting pain relief and improved performance. Objective: To analyze the effects of tDCS combined with Pilates-based exercises in patients with nonspecific CLBP. Methodology: This double-blind randomized controlled trial involved participants of both sexes, aged between 18 and 65 years, with a history of chronic low back pain. Participants were randomly divided into Active tDCS or Sham groups. The protocol consisted of Pilates-based exercises combined with tDCS, performed three times a week over four weeks, with weekly progressions in exercises. The active group received anodal tDCS stimulation on the primary motor cortex (M1) for 30 minutes, while the sham group received only 30 seconds of stimulation. Assessments were conducted at baseline (T0), after the four weeks of intervention (T1), and one month post-intervention (T2). Pain, pressure pain threshold, disability, quality of life, Global Perceived Effect (GPE), medication use, and adverse events were evaluated. Preliminary Results: No statistically significant differences were observed between participants in the active and sham groups regarding pain in the last 24 hours, pain in the last week, pressure pain threshold, disability, and quality of life (in the domains of functional capacity, limitation due to physical aspects, pain, general health, social, emotional, and mental) (p > 0.05). The exception was the vitality domain, which showed significant differences in favor of the active tDCS group (p = 0.02), indicating improvement in this group.
