SUMMARY:

THEORETICAL FRAMEWORK: Among the most prevalent impairments of the shoulder joint are tendinopathies, especially those involving the muscles that make up the rotator cuff. Pain is the most frequently reported discomfort and may be associated with local tissue stress or changes in the central pain sensitization mechanism. Physical exercise is considered the gold standard for the non-pharmacological treatment of these cases. However, considering the characteristics of chronicity and possible changes in central sensitization, other approaches seem to be promising for pain management in this population, such as non-invasive neuromodulation through Transcranial Direct Current Stimulation (tDCS).
OBJECTIVES: To evaluate the effects of tDCS combined with specific isometric and isotonic shoulder exercises on chronic pain in individuals with chronic shoulder tendinopathy.
METHODS: This is a randomized, blinded controlled trial, in which individuals with a diagnosis of chronic shoulder tendinopathy will participate. The primary outcome will be pain intensity and the secondary outcomes will be: physical function, performance, quality of life and global perception of change. Participants will be allocated into two groups: G1 (active tDCS + specific shoulder exercises) and G2 (sham tDCS + specific shoulder exercises) and will undergo three evaluation moments: before, immediately after all sessions and seven days after the last session of the intervention protocol. The protocol will be performed by combining specific exercises for the shoulder joint in addition to the use of tDCS, which will be applied on the Primary Motor Cortex area (M1), with an intensity of 2 mA, for 20 minutes, twice a week, for four weeks. Statistical analysis will be performed using a mixed ANOVA of repeated measures, to investigate the effects of time-group interaction. A confidence interval of 95% and a significance level of 5% will be considered. This research was approved by the Research Ethics Committee (CEP) of the Faculty of Health Sciences of Trairi (FACISA/UFRN) and has the opinion number: 6.055.174.
EXPECTED RESULTS: It is expected that the findings of the present study may contribute to the understanding of a way of managing chronic pain in individuals with shoulder tendinopathies, as well as expanding the scope of knowledge regarding the combination of specific physical exercises for shoulder tendinopathy with the neuromodulation, considering that transcranial stimulation by direct current is configured as a supporting technique in the treatment of chronic disorders and that has been gaining notoriety in recent years.
