IMPROVING THE REPORTING PROCESS OF PATIENT SAFETY INCIDENTS IN A REGIONAL PUBLIC HOSPITAL

SUMMARY:

Introduction: The notification of incidents is an instrument for managing risks and promoting patient safety, allowing learning from errors and planning safe care. However, some factors still hinder the practice of notification in organizations, with emphasis on the low culture of patient safety, which is defined as the set of values, attitudes, skills and behaviors that determine the commitment to health and safety management, replacing guilt and punishment with the opportunity to learn from failures and improve health care. Objective: To increase the quality level of the notification and learning process, based on the number and diversity of patient safety incidents in a regional public hospital. Methodology: Quantitative study with an almost experimental design of the mixed type with before and after and time series analyses. The opportunity for improvement was identified: low number of notifications of patient safety incidents, and later, the analysis of the causes was carried out using the Ishikawa Diagram. The low patient safety culture was classified as an unquantified cause, and its evaluation was proposed as the first intervention of the project. To measure the improvement, non-categorical criteria were elaborated, which use process data and propose to analyze all SP incident notifications per month, not requiring sampling methods. The criteria demonstrated validity and reliability and the dimension studied is patient safety. Results: Of the 12 planned interventions, 6 were implemented, with emphasis on the result of the CSP assessment, which showed as weak points: non-punitive response to errors; problems with shift changes and transitions between units and frequency of notified events. As for the level of quality, the trend graphs show a visible increase in notifications, going from 5 to 24 notifications per month, with monthly regularity, even surpassing the established target. However, two criteria did not change. The context for the improvement was analyzed through the MUSIQ and presented a score 118, indicating that the project can be successful, but faces contextual barriers. Perspectives: It is necessary to sensitize senior management to develop actions aimed at improving the educational culture at the expense of the punitive culture. It is necessary to proceed with measuring the criteria and implementing the planned interventions, in order to assess their impact on the chosen quality problem and achieve the planned improvement.
