RESUMO:

The patient’s records are the means of communication between professionals, users and health services that makes it possible to measure and evaluate the kind and quality of the service provided. Moreover, they contain a series of information about the interventions conducted and help monitor the user’s health history. The adequate documentation of information becomes an important tool for the care and safety of the patient, as well as legal support for the institution and professionals.
The present study has as general objective to evaluate and improve the records in medical records of Hospital Geral de Farias Brito. The research is a study of quantitative nature with a nearly experimental (before and after) drawing of evaluation of quality criteria. The data will be collected from the analysis of the documentation made in the records of the patients hospitalized in the unit. A cycle of improvement with problem analysis, intervention and monitoring will be undertaken. With regard to the type of evaluation, we can say that it is internal, retrospective and crossed. Eight criteria were developed to verify the quality of the records related to patient and assistant identification, legibility and aspects of multiprofessional evolution. The results obtained will be presented through a Pareto chart in which it can be observed whether there was any quality improvement and its significance.
