SUMMARY:

Introduction: Training in Family Practice (FP) involves a complex set of skills. The Brazilian Society of Family and Community Medicine (SBMFC) developed a Competency-Based Curriculum (CBC) in view of the scenario of expansion of FP residency programs in Brazil, in order to guide the training of young family physicians. This work aims to analyze the facilitating and limiting factors in the development of competences during residency in MFC. Methods: This is a qualitative, observational and analytical study, developed with graduates from the Residency Program in Family Practice at Hospital Universitário Onofre Lopes (RPFP/HUOL) from 2016. Two strategies were used: a virtual questionnaire, on socio-demographic data of the graduates and the RPFP/HUOL, with subsequent descriptive analysis of them; and individual interviews with graduates, in order to deepen the understanding of the competences to be developed and how this process took place at the Medical Residency (MR). To assess the material of the interviews, thematic content analysis, based on Bardin, was used, with 3 a priori categories being defined: FP competences to be addressed during MR; Factors that influenced the development of FP competences; Development of competences in communication skills and clinical reasoning at RPFP/HUOL. Results: 7 graduates were interviewed, and 9 responded to the virtual questionnaire, corresponding to a significant part of the 11 residents who closed the RPFP/HUOL in the years 2016 to 2019. In addition to the 3 a priori categories already established, an a posteriori category emerged: Analysis proposal on training for FP. As for the SBMFC’s CBC, it was perceived by the graduates as a document known and applied by the FPRP/HUOL, so much so that the competences mentioned in the interviews are in line with the categories found in the aforementioned curriculum. The contact with the preceptorship and the importance of the preceptor in the development of these competences were very recurrent speeches in all interviews, being denoted as an important facilitator for the development of the resident during the Program. The resident's autonomy was also frequently cited in the sense of being a fundamental requirement for the development of skills and better use of MR, which corroborates with adult learning and brings challenges to the preceptorship and to the planning of the theoretical-practical spaces and evaluative, in addition to the practice scenarios. Conclusions: RPFP/HUOL graduates are, in general, young doctors, graduated from institutions whose curriculum has complied with the National Curriculum Guidelines, and are motivated by the diversity of skills in the specialty. Preceptorship was perceived as a fundamental factor for the development of FP competences. The curricular reorientation of the RPFP/HUOL associated with better structuring of the Health Care Network and teaching-service dialogue, valuing preceptorship, may bring benefits and new challenges to the health system of the municipality of Natal.
