SUMMARY:

Introduction: Training in family and community medicine (CFM) involves a complex set of skills. SBMFC has developed a competency-based curriculum against the backdrop of expanding MFC residency programs in Brazil to guide the training of young family doctors. This study aimed to understand what are the facilitating and limiting factors in the acquisition of skills during residency in CFM. Methodology: This is a qualitative, observational and analytical study with a structured virtual questionnaire applied to CFM residents coming from the Onofre Lopes University Hospital (PRHUOL) residency program since 2016, with questions about socio-demographic data and self-assessment about the acquisition of the core competencies described in the Competency Based Curriculum (CBC) of the Brazilian Society of Family and Community Medicine (SBMFC). Individual interviews were also conducted with PRHUOL graduates, in order to deepen the understanding of the process of acquiring skills in the residence. Results: A total of 7 graduates were interviewed, and 9 answered the virtual questionnaire, corresponding to the residents who closed the program between March 2016 and February 2019. The interview analysis units were grouped into 5 a priori categories (MFC Competencies to be addressed during the residency; Factors influencing the development of MFC competencies; PRHUOL communication skills competences; PRHUOL clinical reasoning competences) and 1 posteriori category (PRHUOL purposive analysis). Discussion: SBMFC's BCC was perceived by MFC graduates as a known and applied tool in PRHUOL, so much so that the skills cited in the interviews are in line with the categories found in the cited curriculum. Contact with preceptorship and the importance of the preceptor in the development of these competences consisted of recurring speeches in all interviews, being denoted as an important facilitator for the resident's development during the program; Resident autonomy is also frequently cited as a fundamental requirement for the acquisition of skills and better use of the residency program, which corroborates adult learning and challenges for the preparation and planning of theoretical spaces. practical and evaluative as well as practice scenarios. Conclusions: The value of preceptorship in the perception of PRHUOL CFM graduates is undeniable, and the extent to which weaknesses related to the number of preceptors and their type of bond directly impact the acquisition of essential CFM skills. The reform of the residency program, with greater clarity of the skills to be achieved during the two years of training, associated with better teaching-service dialogue and structuring of the Natal Health Care Network, and finally the valorization of the preceptor's figure, can bring numerous benefits and challenges to the health system of the municipality.
