RESUMO:

Introduction: Central venous access is an invasive procedure commonly performed in critically ill patients hospitalized in Intensive Care Units (ICU). Several studies have demonstrated that the use of ultrasound (US) to guide venous access increases the success rates of catheter placement, reduces the number of complications and improves the patient safety. In the last decades there has been an increase in the number of studies directed towards the importance of the professional competence formation in medical residency. Aim: To create a training program with active teaching-learning methods in order to develop the competence to perform ultrasound-guided internal jugular punctures. Methods: This is an action research, an exploratory prospective study that involved 43 participants, of these 31 were medical residents, 9 were intensivist preceptors of the University Hospital Onofre Lopes and 3 were preceptors of the Cardiology discipline. Four theoretical-practical courses were taken using a chicken breast model to simulate an ultrasound-guided venous access. Every participant performed pre and post-training tests and was evaluated by the Direct Observation of Procedural Skills (DOPS) on the following areas: area 1: previous knowledge; area 4: technical skills; and area 10: general skills. Results: The evaluation model applied to the participants generated affirmative responses, according to the descriptive results measured by Cronbach's alpha coefficient, which evaluates the consistency and reliability of the responses. Therefore, the training was considered positive by 100% of the participants, who also had a good performance in their majority, and the level of satisfaction regarded to the course was considered great or excellent. In addition to being accepted, this model allowed the assessment of competence in a ultrasound-guided central venous access. Conclusion: The training model using chicken breast is feasible, well accepted and allowed to develop competence in using the ultrasound device as a guide for punctures. The use of DOPS proved to reliable and relevant as an evaluation tool that confers professional authenticity.
