RESUMO:

Introduction: Teaching ambulatory surgery  usually is accompanied by formal and informal methods, combined with activities in ambulatory care clinics, operating rooms and wards, mainly based on observing and performing practical procedures through appropriate and experienced supervision. Teaching strategies comprehend use of molds, sponges, simulators, virtual reality, movies, interactive videos, software, games, responsible use of animals, corpses, human tissues, surgical technique workshops, suture lab and knots. At the proposing institution, the teaching of ambulatory surgery in Primary Health Care is not supported by a specific curricular component.

Objectives: To structure the teaching of ambulatory surgery at the Family Medicine Clerkship (FMC) of the Federal University of Rio Grande do Norte (UFRN).

Methods: The study is characterized as quasi-experimental, exploratory and analytical. It involved all students from the fifth year of UFRN medical school in semesters 2018.1 (control group) and 2018.2 (intervention group) and had a collaborative team of 8 student monitors / actors, as well as 10 preceptors / teachers / evaluators. The methodological course involved several steps such as: structuring the fields of practice; preceptors training; preparation of video lessons; development of standardized silicone rubber bench models for teaching; theoretical and practical class; stimulation the practice of procedures in the practice fields and elaboration of simulated evaluation stations. Instruments based on the Global Rating Scale (GRS) were used to assess students performances at the stations. GRS is an internationally validated scale in which it is possible to identify aspects of students' performance in surgical practice. The evaluation was initially applied to the control group and later to the intervention group. Statistical analysis was performed using Student's t-test for mean variables and Pearson's chi-square test for proportion variables. The significance level of p <0.05 was considered.

Results: From the 2018.1 semester, 42 students (100%) participated in the study, while from the 2018.2 semester, 35 (68.6%) of the 51 were present for the evaluation. The mean age was similar (24.5 years vs 25.3 years, p = 0.497), with male predominance in both groups (59.5% vs 60%, p = 0.996). The overall performance score of the intervention group was 5.44 (SD ± 1.75), higher than the control group which had an average of 3.15 (SD ± 1.47), with p <0.001. When considering the proportion of scores above 5.0 (competent level) in relation to scores below 5.0 (insufficient), we can observe a larger number of students with acquired competence in performing PCA in the intervention group (57, 1%) compared to the control group (9.5%), with p <0.001.

Conclusions: The study largely achieved its objectives by structuring the teaching of ambulatory surgery at the FMC of UFRN through the development of teaching-learning methods and the evaluation of some ambulatory surgical procedures. It has been able to implement and increase minor surgery in most field practice units. Besides noting the gain of competence of the intervention group through comparative analysis.
