RESUMO:

Introduction: According to the National Curricular Guidelines in medicine is imperative that the graduate be able to perform ambulatory surgical procedures (ASP). Their teaching is usually accompanied by formal and informal methods combined with activities in outpatient clinics, surgical centers and wards, based mainly on observation and execution of practical procedures through adequate and experienced supervision. The Global Rating Scale (GRS) is an internationally validated scale in which it is possible to identify aspects of students' performance in the surgical practice. Objectives: Evaluate the competences acquired by undergraduated students who passed through the structured course of ASP, comparing to those who passed before the structuring of the course. METHODS: The study was characterized as quasi-experimental, all the students who had graduated from the ninth period of the medical course of the Federal University of Rio Grande do Norte, in the semesters 2018.1 (control group) and 2018.2 (intervention group) were invited to participate in the study, totaling 77 students. It counted with a collaborating team of 8 monitors / actors, in addition to 10 preceptors / teachers / evaluators. The preceptors received training in order to improve their skills in outpatient surgery and assessment instruments. Simulation stations were designed for the evaluation of surgical skills with standardized silicone material. Instruments based on GRS were used for performance evaluation. The evaluation was initially applied to the control group. For the group of students of the intervention, a course was designed for the development of skills in outpatient surgery for the general practitioner, videos were elaborated and made available for online access, besides the possibility of performing ASP in the health units with the preceptors . At the end of the semester, the intervention group participated in the evaluation with the same stations offered previously to the control group. For the statistical analysis of the data, the student's T test was chosen for the variables of means, and the Chi-square test of Pearson for the variables of proportions. The significance level of p <0.05 was considered. Results: 42 students (100%)  of the 2018 semester have participated in the study, while 35 of the 51 students (68.6%) for the semester 2018.2 were present for the evaluation. The mean age was similar (24.5 years vs 25.3 years, p = 0.497), with predominance of males in both groups (59.5% vs 60%, p = 0.996). The overall performance score of the intervention group was 5.44 (SD ± 1.75), higher than the control group that had a mean of 3.15 (SD ± 1.47), with p <0.001. When considering the proportion of scores above 5.0 (proficient level) in relation to scores below 5.0 (insufficient), a greater number of students with acquired competence in the performance of ASP in the intervention group can be observed (57, 1%) in relation to the control group (9.5%), with p <0.001. Conclusions: With the results found, it is expected that this teaching-learning process can be maintained and be disseminated facilitating the gain of the competences in ASP recommended for the general practitioner. And that the graduate develop autonomy and tranquility to perform the most common procedures in real patients of Primary Health Care, avoiding unnecessary referrals and improving health care.
