THE SIMULATION USE TO IMPROVE COLLABORATIVE PRACTICES AND TEAMWORK IN UNDERGRADUATE MEDICAL COURSE: A TEACHING-SERVICE INTEGRATION STRATEGY.

RESUMO:

This study describes the implementation of the integrated training of the student of the medical school boarding school with the health team during a clinical emergency, using the TOSCE as a strategy to evaluate non-technical skills for collaborative practice and teamwork. This is a quasi-experimental design study to evaluate the technical and human performance gains in CPR using the TOSCE team evaluation tool during OSPE conducted between October 2016 and July 2017. Four workshops were conducted in CPR with duration of six hours, in a skills lab with high fidelity mannequins. Two simulated stations were prepared, one of which was extrahospital (PCRE) and one inhospital (PCRI), in which the participants were grouped into pairs containing a student and a nursing technician. The simulated cases had the objective of learning the immediate decision making, collaboration and communication between the members to fulfill the desired tasks. In addition to performing self-assessment, participants received feedback at each performance evaluation. Results: There was a significant increase in the mean of the cognitive evaluation before and after training, with 5.9 ± 2.7 e7.5 ± 1.8, respectively, p <0.001. Similarly, performance during OSPE also showed a significant improvement in the outpatient PCR station, with median 16 before and 22 after training (p <0.001). The mean scores observed in the in-hospital PCR station (ICRP) were 20.8 ± 9.6 and 30.0 ± 8.9 years and after the intervention (p <0.001). A significant percentage of participants presented a poor initial individual performance, with 63.9% of them in a lower than expected range, 23.2% "well below expectations". This data changed very significantly after the training, presenting a true numerical inversion, with an "inside or above expectations" performance in 63.9% of the individuals and a decrease in the number of participants with the performance and "well below the expected "to 4.8%. The evaluation of the teams showed initially unsatisfactory performance, in which 66% of the pairs had a performance "below" or "much lower than expected", while only 33.3% achieved an "inside" or "over-expected" evaluation. After the intervention, 62.5% of the pairs had a performance considered "in" or "higher than expected" in TOSCE. The answers given in the workshop evaluation questionnaires express acceptance and easy adaptation to the teaching-learning method, although new to a good part of the group. Performing self-assessment during the activity was considered a positive point in a unanimous way, with 25.3% agreeing and 74.7% fully agreeing that the act of reflecting on the performance aided in the improvement. Final considerations: The use of the TOSCE tool was feasible when applied in an emergency scenario as a strategy for the evaluation of training groups, being well accepted by the participants. Its use has enabled us to identify weaknesses and domains of non-technical skills essential to teamwork.
