BACKGROUND: Dementia is a life threatening, incurable and stigmatized condition, with devastating impacts on patient’s personal identity, family and caregivers, being currently one of the most fearsome diseases. There are many barriers to an effective diagnosis disclosure on dementia patients, including fear of causing distress, the degree of certainty of the diagnosis and caregivers objection, besides the lack of training on communication skills in undergraduate medical school. Brazilian elderly population has been under a rapid growth and thus there is a remarkable rise in the number of individuals with Alzheimer disease and other dementias. Therefore, there is an urgent need for implementation of training strategies to help the medical student to build up multiple skills for an appropiate relationship between doctor, dementia patient and family, including the ability of disclosing the diagnosis. OBJECTIVE: To analyse the effect of a protocol for delivering the diagnosis of dementia on the communications skills of the medical student. METHODS: Mixed methods study with students from the 5th semester (Geriatrics clerckship) of the medical school at UFRN, along the 2nd semester 2016 and the first semester 2017. 86 students joined the study, divided into three groups: GI (trained protocol in the beginning of the Geriatrics clerckship), GII (trained protocol after lectures about dementias) and GIII (did not join the protocol). Students on GI and GII were analysed by two similar OSCEs (delivering bad news to an Alzheimer disease patient); students on GIII only took part at the second OSCE. RESULTS: Groups GI and GII showed better results on the second OSCE comparing to GIII (GI:7,56±1,22, GII: 7,47±1,09, GIII: 6,42±1,68, p<0,001). Students on GI performed better on this assessment than students on GII (p<0,001). Groups GI and GII exhibited increasing scores along the two OSCEs (GI OSCE1: 6,38±1,34, GI OSCE2: 7,56±1,22, p<0,001; GII OSCE1: 5,31± 1,36, GII OSCE2: 7,47± 1,09, p<0,001). Regarding students opinions about delivering bad news, 48,2% related feeling unconfortable on this task. The majority (68,1%) of the students that joined the training answered being familiar to SPIKES protocol at the end of the interventions. The students almost entirely agree that a training on delivering the diagnosis of dementia is important for their medical education (98,8%). The analyses of open answer questions revealed the student’s satisfaction for participating of the protocol training and the belief that it will be important for their future occupation. The relevance of practicing communication skills was stressed, especially referring to the OSCE on delivering bad news. CONCLUSION: This training on disclosing the diagnosis of dementia was well accepted by the 5th semester students of the Medical school and contributed for their improvement on communication skills. This model of training, as well as the adapted protocol, may represent replicable strategies for other curriculum elements in a way of improving communicating abilities on health schools.
