ANALYSIS OF STRATEGIC INTERVENTIONS FOR THE QUALITY OF CARE IN THE UNIFIED HEALTH SYSTEM

SUMMARY:

Introduction: the Unified Health System (SUS) has instituted several initiatives to ensure qualification in the Health Care Networks (RAS), although in a partial and fragmented way. Considering the recommendations of the World Health Organization (WHO) for the unification of these initiatives in a National Quality Policy and Strategy (NQPS), it is prudent to analyze whether the management spheres of the SUS (national, state and municipal) have adequately adopted the main strategic interventions suggested to qualify health care. 

Objectives: the general objective is to propose a model for analyzing strategic interventions to improve the quality of care in the SUS, in the current context of fragmentation. Specifically: a) map evaluation studies of national quality policies and strategies in health systems around the world; b) identify available instruments to evaluate strategic interventions for quality of care in health systems; c) describe the quality policies and strategies found in the studies regarding the elements recommended by the World Health Organization and; d) cross-culturally adapt and validate an instrument for measuring strategic interventions for the quality of care in the SUS.

Method: the study has two designs: 1- Scope Review; 2 - Cross-cultural adaptation of a measuring instrument. The scoping review follows the model originally proposed by Arksey and O'Malley, updated by Levac, Colquhoun and O'brien. The search for articles was carried out in the databases Medline/Pubmed, Embase, Cochrane, CINAHL, Scopus and Web of Science, in December 2022. The cross-cultural adaptation consists of the translation, adaptation and validation of the “Questionnaire for Quality of Care Interventions in Health Systems", originally developed in English, for the Brazilian context. It will be carried out between February and June 2024, following the six stages proposed by Beaton et al.

Partial results: there is a scarcity of publications on the evaluation of quality strategies at the health system level. After identifying eighty-five potentially interesting studies, nine met the eligibility criteria. Among the countries that have PENQs, eight are from the African continent, many of them supported by international agencies, such as the WHO. Regarding instruments for evaluating strategic interventions for the quality of care at the level of health systems, four tools were found in the literature. The instrument proposed by Bengoa et al stands out, used to assess the quality of health systems in Afghanistan, Albania, and Mexico. Regarding the elements recommended by the WHO for the implementation of PENQs, the most present were governance and the organizational structure for quality, which were evidenced in all published experiences.

Considerations: the research mapped those interested in developing and evaluating NQPSs and identified the need to develop models and instruments for evaluating strategies for quality in health systems. Future results should propose and analyze a model to evaluate the completeness of quality strategies in the SUS, aiming to strengthen quality improvement in the RAS. The next stage will adapt the prioritized instrument and its results will still be described.
