Introduction: Cardiovascular diseases (CVD) are the main cause of morbidity and mortality in the world. Therefore, strategies for the prevention and treatment of cardiovascular events are studied. Vitamin E stands out for its antioxidant and anti-inflammatory properties, supported by findings that higher dietary intake of vitamin E and higher serum α-tocopherol (α-TOH) concentrations are associated with lower risk of cardiovascular events. Thus, data on the nutritional status of vitamin E in the population and its relationship with cardiovascular risk factors are important strategies to support programs and public policies aimed at preventing and treating CVD. Objective: To evaluate the nutritional status of vitamin E and the relationship with cardiovascular risk factors in adults and older people from the Brazuca Natal Study. Methods: This is a study of different forms: 1) The narrative review on the relationship between vitamin E status and CVD in population studies, based on a search in the PubMed, LILACS, Scielo, Scopus databases, and in the Google Scholar search engine, in all languages, prioritizing articles referring to the period of the last ten years (2012 to 2022). 2) Cross-sectional study, which sought to assess the prevalence of vitamin E deficiency (EVD) and the cardiovascular risk factors that interfere with serum α-TOH status in adults and older people from the Brazuca Natal study. We performed multiple linear regression analysis with serum α-TOH (dependent variable) and cardiovascular risk factors. 3) Cross-sectional study aims to verify the consumption of vitamin E and identify the main food consumed by the evaluated population. We analyzed dietary intake of vitamin E, and socioeconomic and demographic conditions (sex, age, race, education, per capita income, and health district of residence). Results: in the narrative review, we observed that EVD can be a public health problem, as there is a variation of 0.6% to 55.5% of EVD in the world, with higher percentages in Asia and Europe, where high mortality rates due to CVD stand out. Population studies suggest the protective effects of vitamin E in CVD. However, intervention studies with α-TOH supplementation have not confirmed its cardioprotective action. In the cross-sectional study, we observed that 24.8% of adults and elderly people in Natal had EVD, and 89% had low concentrations of circulating α-TOH (below 30 µmol/L). We found that women had higher mean serum α-TOH values. In addition, the cardiovascular risk factors that implied an increase in the serum α-TOH of the individuals were a high index of visceral adiposity and higher values of the global risk score. Study 3 showed that 95.7% of individuals had a low intake of vitamin E, that is, EAR of 12 mg/day. We found lower consumption values in the elderly, women, and people with a per capita income of less than half a minimum wage, and with less education. Soybean oil, açaí pulp, and red meat provided the highest level of ingested vitamin E. Conclusion: The study confirmed a high prevalence of EVD and a low consumption of vitamin E in the population studied. We observed a relationship between serum α-TOH and cardiovascular risk factors.
