SUMMARY:

Introduction: Systemic Arterial Hypertension (SAH) is one of the most prevalent chronic diseases in Brazil and worldwide, and is the main risk factor for the development of cardiovascular diseases. Results of Brazilian Primary Care indicators reveal that blood pressure measurement and consultation in people with hypertension is not being carried out regularly, and it is necessary to study the causes of the problem as well as interventions to improve it. Objective: To analyze the barriers and facilitators for carrying out consultations and regular blood pressure measurement in Primary Health Care as a strategy for improving care. Method: This is an observational study with a qualitative approach based on the technique of content analysis through focus groups carried out in Primary Care in a municipality in the Brazilian Northeast. 29 professionals who work in Primary Care participated in the study. Data collection took place through focus groups that were carried out in two stages. The first stage was carried out with five focus groups in September 2022 and the second stage was carried out with a focus group in May 2023. For content analysis, the Nvivo software was used as support for coding the categories of analysis. Results: The barriers found for regular consultations and blood pressure measurements in people with hypertension are related to units with high demand, lack of records, low qualification of professionals and users with low literacy. To systematize the facilitators, a driver diagram was built with four primary drivers, 16 secondary drivers and 27 change ideas. Final Considerations: In order to improve the results of health indicators, especially the quality of care for people with hypertension, it is necessary to adopt a systemic thinking through a local diagnosis that reveals the causes of the problem and act on them with multifaceted interventions considering aspects such as management capacity, adequate service infrastructure, permanent education of professionals, in addition to community engagement.
