SUMMARY:

Introduction: The success of collaborative quality improvement projects in health depends on the engagement of professionals working in patient care and knowledge and skills in quality improvement methods and tools. The factors that modulate the motivation and assessment of competences in these projects are still unclear. The objective of this study was to analyze barriers and facilitators to the motivation of health professionals in a large project to implement evidence-based infection prevention practices in intensive care units of Brazilian hospitals and to create and validate a tool to assess skills in improvement of quality.

Methods: The study of motivation was qualitative, based on content analysis of semi-structured in-depth interviews with health professionals participating in the collaborative quality improvement project “Improving patient safety on a large scale in Brazil” involving 116 Brazilian hospitals. We randomly selected the hospitals and conducted the interviews via videoconference with the health professionals who participated in the project. The sample was for convenience and the final size was defined by the principle of saturation. Data were coded using NVivo software, and analyzes were based on the Capability, Opportunity, Motivation, and Behavior model and the Model for Understanding Quality Success. The competence assessment questionnaire was constructed after reviewing the literature and the content analysis will be carried out by a panel of specialists using the Delphi technique.

Results: For the assessment of motivation, 28 health professionals from 12 hospitals in the five regions of Brazil were interviewed. The barriers found were the non-involvement of top management, demand for immediate results, lack of inputs, excessive bureaucracy for changes, work overload, lack of workload foreseen for the project, resistance from experienced health professionals, belief that the improvement increases workload, difficulty engaging night shift professionals, little involvement of physicians, lack of knowledge about quality improvement and indicator collection. Active participation of top management in the rounds, institutional policy of continuous improvement, quality improvement team constant presence in the unit, enthusiasm of the team leader, understanding the reason for changes, teamwork, participating in change tests, learning in practice, celebrating small results and feedback on the results were facilitators for motivation. The competence assessment questionnaire is yet to be validated, but we observed that the collaborative project contributed to the training of participating professionals, enabling a practical experience in the quality improvement method.

Conclusion: This study reinforced the importance of customizing large projects, involving motivation modulators, intervening in factors identified as barriers and facilitators, and verifying interactions in the microsystem to ensure maximum motivation during collaborative projects. And the expectations for the contributions of the second study are high due to the possibility of closing the link between motivation and skills necessary for the success of collaborative projects.
