Introduction: The food environment can be defined as the context in which the individual is inserted, where food choices are influenced by access to food, availability, price and quality. Disadvantageous environments, with low availability, high costs and low quality of healthy foods result in poorer diet quality, especially in areas where food insecurity is more prevalent. Objective: To analyze the food environment and its relationship with the occurrence of food insecurity at home and socioeconomic conditions. Method: This is a study of different methods. 1) For the Systematic Review of the relationship between food environments and food insecurity in adults and older adults, we used the acronym PICOS and the Embase, PubMed, and LILACS databases. We followed the PRISMA 2020 guidelines, and the languages were limited to Portuguese, English, or Spanish, and no restriction on the period of publication. Protocol registered in PROSPERO with registration CRD42021239145; 2) The ecological study describes the community food environment and its association with socioeconomic conditions by neighborhoods in Natal/RN. Information on the type of establishment that sells food and geographic coordinates (latitude and longitude) were collected from data from the Food Sanitary Surveillance of the city of Natal, Google Maps and Laboratório Horta Comunitária Nutrir. The socioeconomic data of the neighborhoods were collected from the 2010 Census. The establishments that sell food were grouped into healthy, when they sold predominantly minimally processed foods; unhealthy when ultra-processed products are predominantly sold; or mixed, with sale of both. Rates for the categories of these establishments were calculated per 10,000 inhabitants. We used QGIS software version 3.26 to do point mapping and heatmaps. We evaluated the relationship between the rate of ultra-processed establishments and neighborhood socioeconomic conditions using multiple linear regression analysis. 3) Cross-sectional study to assess the association between the establishments that sell food described in article 2, food insecurity at home and socioeconomic conditions, among the participants of the Brazuca Natal Study. This study will analyze the relationship between the food environment, the prevalence of food insecurity, and the socioeconomic conditions. We selected 411 adults and older adults by two-stage cluster probabilistic sampling in all administrative districts of Natal/RN. QGIS software version 3.26 will be used for the analyses. Results: Thirteen articles were included in the systematic review, using different methods to measure the food environment. Nine studies found that the food environment is associated with food insecurity. In the ecological study, we identified a higher density of establishments selling unhealthy foods (51%) and only 8% healthy ones. There was also a greater concentration of establishments selling food in the richest neighborhoods of the city. A higher rate of establishments selling ultra-processed (unhealthy) foods was found in neighborhoods with higher average income (p<0.05). Conclusion: A food environment with low access and availability of healthy foods is related to a higher occurrence of food insecurity. There are socioeconomic inequalities in the distribution of different food establishments in Natal/RN, with a higher density of establishments in the more central and richer areas of the city.
