INTRODUCTION: The subject of quality has become a permanent agenda within the Unified Health System in the last two decades. Among the implemented strategies, there is the National Program for Improving Access and Quality in Primary Care (PMAQ AB), relating to a set of strategies to strengthen the National Policy on Primary Care (PNAB), in addition to involving the participation of managers , health professionals and service users. OBJECTIVE: The study aims to analyze the quality of maternal and child care in the context of Primary Care in Rio Grande do Norte in the three cycles of the National Program for Improving Access and Quality from the perspective of users. METHOD: This is an evaluative, cross-sectional, retrospective study using secondary data from the 1st, 2nd and 3rd Cycles of the National Program for Improving Access and Quality of Primary Care (PMAQ-AB), whose sample consisted of 7,787 users, as well as, respectively, 1,085, 715 and 866 Basic Health Units participating in the external evaluation, according to the Cycles. Four maternal-infant indicators were used as dependent variable, with a view to validating the quality improvement in Primary Care. A descriptive analysis of the variables was performed, as well as the Kruscal-Wallis test to compare the means of the indicators in the 3 cycles of evaluation of the PMAQ. The variables that showed statistical significance (p<0.001) had the hypothesis confirmed. RESULTS: In the organizational dimension, when analyzing the quality of care, a significant difference (p<0.001) was identified in the sub-dimension of women's health - access to health services between the 3 cycles, with an average of 64.98% in the last cycle. cycle. Regarding satisfaction, 92.13% recommend the Basic Health Unit to a friend, friend or family member (p<0.001) and only 8.77% (p<0.001) of users would change team or Basic Health Unit if they had the option. However, the sub-dimensions quality of puerperal consultation (5.96%) and quality of child care in the first week of life (6.37%) had their results classified as unsatisfactory (≤25%) in the 3rd Cycle of the Program. The structural dimension showed improvement in the three sub-dimensions, inputs 96.76% (p<0.001) and material 92.35% (p<0.001). It is also considered to evaluate the correlation between the quality and satisfaction indicators of the users and the maternal-infant indicators of the municipalities of RN.
