The conceptual complexity of Food and Nutritional Security (FNS) makes its assessment a challenge, requiring the use of complementary indicators, capable of covering the food and nutrition components and their different dimensions: availability, access, stability, and biological use. Despite this, the Brazilian Food Insecurity Scale (BFIS), the main instrument used to measure Food Insecurity in Brazil, includes only the food component and the dimension of access to food. The formulation of a global indicator is necessary to optimize screening and to identify individuals at risk of food insecurity. Thus, the present study aimed to assess the factors associated with food and nutritional insecurity and to develop a multidimensional instrument for screening. This is a cross-sectional study, conducted with 411 adults and elderly people domiciled in the city of Natal, members of the Brazuca Natal study. Sociodemographic, lifestyle and health issues, and the perception of food insecurity were investigated. For this, interviews were conducted with the application of an electronic questionnaire to obtain socioeconomic data, as well as anthropometry (weight, height, body perimeters), food consumption (Scale of adherence to Dietary Guidelines of the Brazilian Population), and food insecurity (through BFIS). The descriptive analysis was presented by absolute and relative frequencies. prevalence and confidence interval (95%) were calculated for presents Food Insecurity. The analysis of factors associated with food insecurity was performed using Poisson Regression, with a robust estimate of variance, to obtain the gross and adjusted prevalence ratios (PR) and their confidence intervals (95%). Variables with significance up to 0.20 in the bivariate analysis were included in the model. Those with significantly less than 0.10 remained in the Poisson regression. The option for Poisson Regression instead of Logistic Regression was due to the understanding that its use is more suitable for cross-sectional studies, with high prevalence outcomes. The predictive power of food and nutritional insecurity indicators was assessed using the ROC curve, from which to determine the cutoff points for classifying food and nutritional insecurity. The population was mostly represented by women (57.9%), adults (53.5%), black (51.3%), and married or in a stable union (62.8%). The prevalence of FI was 42.1% (37.4% - 46.9%) of the interviewees, with 26% mild FI, 9.2% moderate FI, and 6.8% severe FI. Women (47.5%), adults (48.2%), and people of black color/race (52.7%) had significantly higher FI than the other strata. Having children under 18 at home (PRaj = 1.4; 1.1 - 1.7), main income below R$ 1996.00 (PRaj = 2.3; 1.5 - 3.5), low schooling (PRaj = 1.4; 1.2 - 1.7), inadequate drinking water treatment (PR = 1.4; 1.2 - 1.7), the presence of chronic non-communicable diseases (PR = 1.3; 1.1 - 1.7), not eating fruit for breakfast (PRaj = 1.9; 1.4 - 2.7), eating meals on the living room sofa or in bed, skipping at least lunch or dinner and “pinching” in-between meals (PRaj = 1.3; 1.1 - 1.6) were the variables that best explained the occurrence of FI after multiple analysis, being then listed to compose the multidimensional instrument.  The proposed instrument was able to cover indicators of the dimensions of access, stability, and biological use, for tracking food and nutrition insecurity in adults and the elderly.
