Essential medicines are those that must meet the priority health needs of a population. Population aging is a growing reality in the world and the elderly have the need to use medications, often exposing them to risks. Potentially inappropriate medications (PIM) for the elderly are those where the risks outweigh the benefits in this age group. The aim of the study was to analyze the use of medications by the elderly, based on the selection of essential medications and prescription in Primary Health Care (PHC). To this end, a descriptive and analytical study with a qualitative and quantitative approach was carried out, from March to December 2019, at PHC in the municipality of Campina Grande, PB. The research was developed based on three axes: 1) documentary research, which analyzed RENAME's medications in the 2010, 2013 and 2020 editions according to the AGS / Beers lists, the Brazilian Consensus on Potentially Inappropriate Medicines for the Elderly (CBMPI) (RENAME 2020) and the World Health Organization (WHO) Essentials Medicines Lists (EML) at the time; 2) cross-sectional study, conducted through interviews with 458 elderly users of 71 basic health units. The independent variables covered demographic and socioeconomic characteristics, health condition and use of medications and the dependent variable was the prescribed medication to be classified as PIM in the CBMPI. Descriptive data analysis and Poisson regression were performed; and, 3) interviews with 10 PHC prescribers to learn about the perception of these professionals about the prescribing practice of medications for the elderly. There was an increase in the presence of PIM in RENAME editions from 5.0% (14/277), in 2010, to 9.0% (37/411) in 2020; and from 30.8% (4), in 2010, to 35.1% (13), in 2020, of those who were not part of the EML. Most PIMs have been offered by the Basic and Specialized Components of AF. According to the CBMPI, 12.4% (51) are PIM. 1449 drugs were prescribed, of which 91.6% comprised RENAME and 19.3% were PIM. Of these, 73.5% (205) made up the national list and the majority had operations in the Central Nervous System (CNS). Few therapeutic alternatives were observed at RENAME. In the cross-sectional study, a prevalence of prescription of at least one PIM was identified in 44.8% of the elderly. In the adjusted model, depression (PR = 2.01; 95% CI 1.59-2.55), using other medications in addition to those prescribed (PR = 1.36; 95% CI 1.08-1.72) and polypharmacy (PR = 1.80; 95% CI 1.40-2.33) remained a factor associated with the use of PIM and self-reporting having systemic arterial hypertension became a protective factor (PR = 0.65; 95% CI 0.49- 0.87). There was an increase in the presence of PIM at RENAME and there was a widespread use of PIM for the elderly cared for in PHC. There is a need for actions that seek to qualify access to medicines by the elderly, such as the training of prescribing professionals and the monitoring of the elderly. In view of the aging of the Brazilian population, it is pertinent to guarantee access combined with the safety of medicines for the elderly.
