EVALUATION OF MENTAL HEALTH CARE PROVIDED TO PERSONS WITH MENTAL DISORDERS IN CONFLICT WITH THE LAW IN THE STATE OF RIO GRANDE DO NORTE

RESUMO:

This is an exploratory and descriptive evaluation study whose general objective is to evaluate the mental health care provided to people with mental disorders in conflict with the law in the state of Rio Grande do Norte. The specific objectives are: to verify the evaluation of the National Policy for Comprehensive Health Care for Persons Deprived of Liberty (PNAISP) and the National Mental Health Policy (PNSM) regarding the assistance provided to people with mental disorders in conflict with the law and propose an evaluation model; construct and validate an evaluation tool for mental health care offered in the prison environment; and analyze the assistance provided to this clientele according to the perspective of managers and health professionals, considering the proposed and validated criteria. To this end, the study is being developed in the following stages: 1) Methodological research, with a quantitative approach, for the construction and validation of an instrument for assessing mental health care offered to persons in conflict with the law, which was systematized according to adaptations of psychometry, from the stages: I- theoretical procedures, to identify the contents that composed the proposed instrument; II - empirical procedures, with the validation of the content of the instrument, carried out with experts from the area of mental health and prison health, through a form built using the Google Forms tool; and III - analytical procedures, when the validation will be analyzed, using the Delphi technique, in order to confirm or refute the validation of the proposed instrument; 2) Case Study to be carried out in the Psychiatric Unit of Custody and Treatment of Rio Grande do Norte, with managers and the health professionals who attend this service. This research was appraised and approved by the Research Ethics Committee, through the Opinion of 3084.950, dated 12/14/2018, CAAE nº 01975418.9.0000.5292. A theoretical essay was 

carried out guided by the discussion of the total institutions of Erving Goffman, which analyzed the existence of invisible and invisible walls that bar people with mental disorders in conflict with the law. Later, a scoping review was conducted by the JBI Institute Reviewer's Manual, with the objective of identifying and mapping the mental health care offered to the inmates of the Forensic Psychiatry Institutes in Brazil, whose results identified that the studies on the subject were predominantly dissertations , published in the year 2014, carried out in the southeastern region of Brazil. Qualitative research was emphasized in the area of Psychology and Law, with emphasis on the speeches of health professionals and who did not specify the methodology of data analysis. Besides indicating that the assistance offered to inmates of these institutions is punitive, it is based on excessive medicalization and diverges from current legislation, which makes it difficult for the social reintegration of these subjects. The PNAISP evaluation study identified that it is evaluable, however in the scenario of this study, it is not sufficiently implemented, as well as enabled the elaboration and validation of the logical model, the comparison between such model and the reality of the implementation of the policy to be evaluated and the construction of the matrix of indicators and evaluation questions. Preliminary results indicate that mental health care offered to people with mental disorders in conflict with the law is permeated with stigmas and prejudices, and that the current Brazilian legislation is marked by gaps in the care of this clientele. Therefore, it is recommended to incorporate specific monitoring and evaluation instruments for this policy.
