The black population is identified by several studies as a vulnerable population because it is disadvantaged in socioeconomic aspects, morbidity and mortality profile and access and health services. The inability of society's institutions, structures and organizations to meet this population equanimously can be evidenced in the difficulty of access to health services, such as through the practice of racial discrimination perpetrated by the professionals that compose these services. This study aims to analyze the discrimination and access to health services by the black population in Brazil, based on data from the National Health Survey (PNS) of 2013. This is a cross-sectional study that used data from the National Health Survey PNS) - 2013. The following survey questions were used: "Have you ever felt discriminated against or treated worse than other persons in the health service by a physician or another health professional? ", we selected those who answered" yes "to racial discrimination; and "J17. In the last two weeks, why did not you go to the health department? "" In the last two weeks, participants who answered "no" to the first question and had the answer to the second question "had no money", "the place was distant or difficult to access", "incompatible schedule", "care is very time consuming", "The establishment did not have a specialist compatible with their needs", "felt that they had no right", "did not like the establishment's professionals", "health services strike" and "transportation difficulties". Descriptive analysis of the variables, bivariate analysis of discrimination and access with socioeconomic characteristics, and multivariate analysis were performed using Poisson regression, with Wald test for robust estimation, considering the 95% confidence interval. The prevalence of racial discrimination was higher among black people, aged 25-39, who are smokers, who have four or more morbidities, evaluate their health as bad or very bad, are users of the public health service, live in urban areas . The difficulty of access to health services is concentrated in specific regions of the country, especially in rural areas, victimizing the black population (black or mulatto), who use SUS and have a poor evaluation of their health, even though they do not have morbidities. We found that racial discrimination and the difficulty of access to Brazilian health services reach the black population in the majority. We observed the legitimacy of institutional racism through Brazilian health services, with emphasis on the Unified Health System (SUS), which is proposed universal, inclusive and integral. We reinforce the need to strengthen health policies with a view to transforming this situation.
