SUMMARY:

Abstract

Introduction: Physical function represents an important marker of health outcomes during the aging process. In women, it appears to be influenced by hormonal changes that occur during menopause. However, studies evaluating the association of physical function with menopausal status or the use of hormone replacement therapy (HRT) present conflicting results. Understanding these relationships is important for planning and directing practices aimed at preventing disability in this population. Objective: To evaluate the association between menopausal status and hormone replacement therapy (HRT) on physical function in middle-aged and elderly women from different socioeconomic backgrounds. Methods: Three studies make up this thesis. The first is a systematic review on the associations between menopause status and physical function in women. Observational studies that have assessed menopause status and physical function in different regions and socioeconomic contexts of the world will be included. The review followed a protocol previously prepared and registered on the Prospective Register of Systematic Reviews platform. The second study is a cross-sectional survey using data from the Canadian Longitudinal Study on Aging (CLSA), with 12,506 postmenopausal women. Those who use or used HRT were compared with those who never used it in relation to measures of grip strength, gait speed, timed up and go (TUG), sit-up and balance. The third study will be an analysis of the National Health Survey, with postmenopausal women, which will evaluate the history between the report of HRT and difficulty in carrying out basic activities of daily living (BADL's). Preliminary results: The article systematic review protocol was published in a peer-reviewed journal and the review article is in the final stages of preparation. 21 studies from different contexts were identified, with articles covering populations from high-, middle- and low-income countries. The results indicate that women in both natural and surgical postmenopausal, with and without the use of HRT, have lower performance compared to women in pre- or perimenopausal. Furthermore, it seems that the context is related to these differences, with women from regions with a lower level of socioeconomic development having the worst results. The result of the second study shows that HRT was associated with better physical performance for gait speed and TUG, highlighting the benefits in physical function for women who used or are using HRT compared to those who do not use it, strengthening the idea that this is a likely efficient alternative to delay the damage inherent to the female aging process. The third study is in the data analysis phase and does not yet have results.
