SUMMARY:

Parkinson's disease (PD) is a chronic, progressive, degenerative and disabling disease. With their evolution, motor deficits respond progressively less to dopaminergic therapy, representing an unresolved therapeutic challenge. Thus, we seek to analyze the effects of complementary therapeutic alternatives such as Transcranial Direct Current Stimulation (ETCC) and Binaural Vibro-Acoustic Brain Stimulation (ECVAB) and their impacts on rehabilitation in PD. The aim of this study is to compare the use of transcranial direct current stimulation with that of binaural vibroacoustic brain stimulation in the rehabilitation of patients with Parkinson's. This is a randomized, controlled, double-blind clinical trial with a 60- day follow-up with an estimated sample of 42 patients, of both sexes, aged between 55 and 70 years, recruited by convenience, divided into group G1 , treated with Transcranial Direct Current Stimulation (ETCC) followed by Treadmill Gait Training (TME); and G2, treated with Binaural Vibro-Acoustic Brain Stimulation (ECVAB) and TME. Participants are assessed through: clinical and sociodemographic assessment; Hoehn and Yahr scale (HY); the UPDRS scale (1 – Mental status; 2 – Activities of Daily Living; 3 – Motor exploration; 4 – Complications); tremor analysis by a digital vibrometer; postural and gait kinematic analysis by KINOVEA. Intervention groups receive 10 sessions, twice a week for 5 weeks. tDCS is applied using 35cm2 electrodes, positioned on the cerebellum (anode) and on the CZ area (cathode), with a current of 2mA for 20 minutes. The ECVAB consists of the application of a vibro-acoustic frequency produced and conducted by a high-definition digital bone conduction phone, model (VG02) for 20 minutes, with the subject blindfolded, in an acoustically neutral environment, receiving a Binaural Brain Stimulation of 10Hz, (medium frequency of alpha band), negative polarity. TME consists of a 5-minute warm-up walking on the treadmill at a minimum speed of 01 (equivalent to 0.7m/s); followed by 10 minutes on the treadmill at a comfortable maximum speed; finishing with 5 minutes of recovery (minimum speed of 01). The forecast risks for the application of tDCS and ECVAB are minimal, rare and transitory; and all participants will receive instructions accordingly. TME can generate physical fatigue, which will be minimized with the immediate suspension of the procedure. The research will bring the benefit of offering an innovative treatment for rehabilitation in PD, stimulating their related motor skills and will offer important information to the patient about an overview of the staging of their disease and their motor functions. From the data tabulated and analyzed in the SPSS 22.0 program, descriptive analysis will be carried out to characterize the participants, and the mixed analysis of variance (ANOVA) of repeated measures will be applied, attributing significance of 5% in all tests. The results of this research are expected to find evidence for Parkinson's treatments using ECVAB and tDCS, highlighting, if identified, the most effective method between both. The research is already in the initial phase of collection, with initial data already available, showing positive results in both techniques, however, still without definition, due to still insufficient data for a more precise analysis. If the objectives of this study are achieved, this work will make it possible to suggest a gold standard non-invasive neurostimulation therapy in the rehabilitation of Parkinson's patients.
