SUMMARY:

Amyotrophic Lateral Sclerosis (ALS) is a multisystemic neurodegenerative disease with clinical, genetic and neuropathological heterogeneity, with cerebral impairment, characterized by reduced lung volumes and ineffective cough, its main cause of morbidity and mortality. Mechanical insufflation-exsufflation (MI-E), also known as 'cough assist', consists of increasing the expiratory air flow and, thus, promoting an increase in the peak cough flow (Cough) and the removal of secretions. Objective: To analyze and describe the acute effects of a single treatment of I-EM on peak cough flow (PCF) and chest wall operating volumes (CW) in individuals with ALS versus healthy subjects. Methods: This is a cross-sectional analytical study composed of two groups: an experimental group formed by individuals with ALS (GEla) and a control group (CG) formed by paired healthy individuals. Where pulmonary function, respiratory muscle strength, PCF and PT volumes are evaluated, before (PreMI-E) during (MI-E) and after the application of the MI-E technique (PostMI-E), through Optoelectronic Plethysmography (OEP). Results: Nine GEla participants of both genders (55.9 ± 15.3 years old and BMI 23.2 ± 4.4 kg/m2, FVC 2.62 ± 1.0 L, FEV1/FVC 0,79 ± 0.1 L), with a predominance of clinical presentation of the disease in its spinal form (55.5%) and mean time living with the disease of 26.1±13.8 months. The variables used as standard analysis parameters seem to demonstrate a slight improvement after the application of the protocol, while the PCF had a slight increase. The fast and superficial running index seems to demonstrate a reduction after using the I-EM equipment. Conclusion: According to the preliminary results, it can be concluded that the study findings are similar to those found in the literature, however it is necessary to carry out further analyzes in order to characterize the thoracoabdominal pattern through the contributions of the different compartments in the distribution of CW volumes before and after the application of MI-E, as well as to investigate the changes in lung volume recruitment and adaptation of controlled muscles in response to the MI-E technique.
