SUMMARY:

Background: Although there is no cure, asthma can be controlled by adopting adequate monitoring and treatment measures. Among these strategies, the assessment, support and routinely monitoring of asthma control are essential aspects to identify treatment needs, thus, minimizing disease repercussions. Besides the existing questionnaires translated and adapted for Brazilian pediatric population with asthma, they present limitations related to the lack of structural adequation for its applicability in different age ranges among childhood to adolescence. Objectives: To provide novel instruments for assessing, supporting, and monitoring pediatric asthma control for Brazilian population. Methods: To achieve the proposed objective, we are conducting three studies. Study 1 comprises a systematic review of measurement properties of the existing measures for asthma control assessment. The study methods follow the Consensus-based Standards for the selection of health Measurement Instruments (COSMIN). Study 2 comprises the process of translation, cross-cultural adaptation, and evaluation of measurement properties of the Pediatric Asthma Therapy Assessment Questionnaire (ATAQ) for the Brazilian population. The following study is being conducted according to international recommendations for this type of study. Study 3 comprises the development and validation of an electronic system for Assessing, Supporting, and Monitoring pediatric Asthma Control (e-ASMA), including measurement instruments in health (questionnaire and diary), a mobile app, a web system, and a public portal. The development and analyses of the measurement properties of these instruments are conducting according to COSMIN. In parallel, we are developing the mobile app in React Native, and the web system, and the public portal in PHP. Preliminary results: In study 1, we included 79 studies related to 37 measurement instruments. In study 2, the translated questionnaire (i.e., instructions, items, and response options) was considered adequate by the multidisciplinary committee of experts (Kappa Fleiss’s < 0.70) and by the target population (30 parents/caregivers of children/adolescents with asthma between 5 and 17 years). In study 3, we are carrying focal groups discussions with the target population (i.e., children/adolescents and parents/caregivers) for developing the theoretical conceptual model that will be used for item generation. Preliminary conclusions: The systematic review identified that several measurement instruments were developed to assess asthma control in limited age ranges. These instruments use different response scales and most were developed in English. The Brazilian version of the ATAQ is cross-culturally adapted, and we are currently carrying the analysis of the measurement properties. The qestionnaire and diary that compose the e-ASMA system are in the phase of concept elicitation (focus group) and construction of the electronic prototype of the mobile application.
