RESUMO:
Introduction: The effects of physical training in patients with chronic heart failure (HF) are generally evaluated by changes in peak oxygen uptake (VO2peak). However, despite being widely used in the literature, it has some limitations, such as the need for maximum exercise, which is not possible in severely limited patients. Other variables that do not have these limitations because they are employed independently of maximal exertion have been studied to identify the prognosis of these patients, such as Anaerobic Lumbar - LA, VE / VCO2Slope, oxygen consumption efficiency - OUES and OV - Ventilatory Oscillation and more. recently to evaluate the effects of exercise. Objective: To evaluate the independent variables of maximal effort (AT, VE / VCO2slope, OUES and OV), as well as risk stratification and assessment of the metabolic equivalent of the task in detecting the gains of physical training in cardiorespiratory capacity in patients with chronic HF. Materials and methods: This is an uncontrolled clinical trial composed of patients with chronic HF. The subjects underwent an initial assessment through the Cardiopulmonary Stress Test (CPET), where the VO2peak variables and the independent variables of the maximum effort were evaluated. They were then submitted to a training protocol, consisting of aerobic and resistance exercises, performed 3 times / week, lasting 60 minutes each session, for 12 weeks, and reassessed at the end of the training period. Preliminary Results: The present study identified a 15.2% increase in VO2peak after training, as well as a 12.9% increase in VO2LA, 19.2% in OUES, a reduction in the mean LV / VCO2slope and a reduction in the presence of OV. . In addition, by analyzing the sensitivity and specificity of the variables compared to VO2peak through the Roc curves, areas under the curve greater than 0.70 for VO2LA, OUES 75% and 90% were found, proving that they are also efficient in identifying the effects of VO2peak. physical training as well as VO2peak. A better prognosis was also observed in patients with a change in risk stratification and higher values in the metabolic equivalent of the task, which may also be important indicators in the assessment of CR.
