Abstract:

The overall purpose of the doctorate study is to evaluate the relationship between Myofascial Trigger Points (MTrPs) and Pressure Pain Threshold (PPT) and clinical variables of pain, mobility, and strength in individuals with unilateral Subacromial Impingement Syndrome. The results of this project will be presented in two studies: 1) Relationship between Myofascial Trigger Points and Pressure Pain Threshold and pain, mobility, and strength of shoulder in individuals with Subacromial Impingement Syndrome; 2) Applicability of pressure pain threshold on kinetic-functional changes from treatments for non-traumatic shoulder pain: a literature review protocol. The first is a cross-sectional study that evaluated 39 individuals (23 men e 16 women; 34,71±11,71 years old; BMI de 24,89±3,06 kg/m²). All individuals were evaluated bilaterally for the presence of MTrPs (upper and lower trapezius, infraspinatus, supraspinatus, minor pectoral and middle deltoid), PPT (upper and lower trapezius, infraspinatus, middle deltoid and tibialis anterior), range of motion - ROM (sagittal and scapular flexion, internal and external rotation), isometric strength (scapular flexion, internal and external rotation), and pain during ROM and strength. For comparative analysis between the sides, we used the Wilcoxon test (MTrPs, PPT, ROM, pain during ROM, and pain during the strength test) and Student's t dependent test (isometric strength). The Spearman correlation test (r s) was used to evaluate the relationship between the amount of MTrPs and PPT with ROM, pain during ROM, strength and pain during the strength, adopting a significance value of p <0.05. There were weak and positive correlations between active MTrPs and sagittal flexion ROM, internal and external rotation, and the strength of scapular flexion and external rotation. Correlations between weak to moderate and positive were found between PPT and the strength of scapular flexion. The second study is a literature review protocol with the purpose of examining the current evidence on PPT regarding procedures for evaluation, reliability, and responsiveness to treatment for non-traumatic shoulder pain. Observational studies and clinical trials will be included if they reported: procedures of PPT training and evaluation; reliability and error reporting of PPT measurements of examiners in the studies; the validity of the measure and the PPT of interventions; PPT responsiveness to other variables of shoulder pain, mobility, strength, and function. A critical analysis will be carried out on the use, applicability, precautions and limitations of PPT in the physiotherapeutic evaluation and its associations with the other clinical variables.
