RESUMO:

Introduction: The presence of preserved axonal pathways below the injury level in individuals with complete spinal cord injury (SCI) has been demonstrated through surface electromyography (sEMG). However, little is known about the transverse and longitudinal behavior of this signal in response to the sensory afferent load associated with the supraespinal descending commands during a voluntary attempt to take steps. Objectives: 1) To investigate the behavior of the sEMG signal during an attempt to move the lower limbs according to the verbal inputs related to gait kinematics during passive standing in subjects with SCI; 2) To determine the longitudinal behavior of the EMG signal during passive standing and robotic gait associated with the attempt to voluntary move the lower limbs. Methods: This was an observational analytical study in which 3 subjects with complete SCI in the chronic phase (more than 1 year after injury) participated. The sEMG signal of muscles below the injury was recorded longitudinally during two modalities of the rehabilitation care intervention program. In the first phase, the sEMG signal was recorded in the passive standing associated with the attempt to take steps according to 4 verbal inputs referring to the kinematic characteristics of the steps (FAST, SLOW, HIGH, and LONG). In the second phase, the sEMG signal of the same muscle was recorded during robotic orthosis-assisted gait training (Lokomat®) concomitant with the attempt to take steps according to 2 verbal commands (HIGH and LONG). The transverse and longitudinal behavior of the EMGs signal for the rectus femoris, femoral biceps, anterior tibial and medial gastrocnemius muscles was analyzed bilaterally, identifying its temporal and spectral properties for each experimental condition. The electromyography signal was rectified  and filtered and the linear envelope was presented graphically for each muscle evaluated. The representation of the spectral characteristics over time was determined by the spectrogram. From the identification of muscle contractions, duration, RMS, peak of amplitude, mathematical integral and median frequency of the EMG signal of each experimental condition were determined. Friedman test was used to compare the dependent variables between the different commands and over time. Expected Results: It is expected to characterize the signal sEMG of muscles below the lesion in individuals with complete SCI in order to understand the behavior of this signal by sensorial afferent loading and concomitant supraspinal descending command. It is believed that there are residual supraspinal neural pathways in patients with clinical diagnosis of complete SCI and the knowledge of their properties are considered important to propose new protocols and lines of research for evaluation and intervention in these patients.
