RESUMO:

Duchenne Muscular Dystrophy (DMD) and Becker (DMB) are the most common forms of myopathy affecting children. Respiratory muscular involvement in both pathologies leads to muscle weakness and reduction of pulmonary expandability, with retention of secretions, due to the inability to generate an effective cough, leading to morbidities such as pneumonia and respiratory failure. As respiratory muscles are activated to changes in body positioning to generate an effective cough in these individuals has not yet been explained in the literature. Objective: To evaluate and compare the acute effects of different postures on variations in cough peak flow (CPF) and electromyographic activation of respiratory muscles in individuals with DMD and BMD. Methods: CPF, distribution of tidal volume (VT) in the chest wall compartments (rib cage and abdominal) were assessed by optoelectronic plethysmography and activation of the respiratory muscles (sternocleidomastoid, scalene, abdominal rectus and external oblique) through the surface electromyography (EMGs) in 3 position adjusted to one of two different inclinations (~80,~45 with respect to the floor) and supine on a rigid flat bed, in subjects with DMD versus subjects with BMD. Twenty volunteers (12 excluded) were evaluated, 5 of them with DMD and 3 with BMD. Children with DMD presented lower values of pulmonary function and maximal respiratory pressures. A significant increase in PFT in position 80o was found when compared to the position at 45o and supine (9.53 ± 6.27 L / s vs. 6.08 ± 3.43 vs 5.14 ± 2.87, p <0.001, respectively) with greater activation of the inspiratory muscles at 80o. In addition, there was an increase in the contribution of ribe cage to the VT of the chest wall at positions supine, 45o and 80o in subjects with DMD [42.24 (34.29-58.47) vs 42.14 (25.30-53.67) vs 61.34 (44.69-68)] and with BMD [45.78 (33.77-53.08) vs 49.8 (46.09-66.70) vs 64.90 (47.36-71.55)], respectively. Our preliminary results suggest that cough is favored in position 80o with better inspiratory muscle activation in patients with DMD and BMD.
