IMMEDIATE EFFECTS OF NIV ON PERIPHERAL MUSCULAR FUNCTION AND AEROBIC PERFORMANCE IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE: A RANDOMIZED CLINICAL TRIAL

RESUMO:

Introduction: Although it is a primarily pulmonary disease, the natural history of Chronic Obstructive Pulmonary Disease (COPD) is associated with numerous systemic manifestations and comorbidities that complicate its evolution. Of these, dysfunction of skeletal muscle stands out for entailing a greater impairment in functional capacity, low tolerance to exercise and reduction of the quality of life of the individual. Noninvasive ventilation (NIV) during exercise may improve respiratory muscle function and consequently exercise tolerance. Aim: To evaluate the acute effect of NIV in the bilevel mode, on peripheral muscle function during isokinetic exercise and aerobic performance through the 6-Minute Walk Test (6MWT) in patients with COPD. Methods: a clinical trial to be performed with COPD patients from 50 to 80 years old, who will be recruited from the pulmonology service of a University Hospital of high complexity and Basic Health Units, both in the city of Natal/RN. All subjects will be submitted to the functional capacity evaluation, with 6MWT, before being submitted to the evaluation of muscular function; the latter being composed of the variables of peak torque, total work, fatigue index, power and strength, through the isokinetic dynamometer and effort perception scale, of the Femoral Quadriceps muscle of the dominant limb. Prior to such assessment, subjects will use NIV (Bilevel Pressure Airway Positive). Blood samples of lactate, lactate dehydrogenase and creatine kinase will be collected to evaluate fatigue and muscle damage. Expected results: To understand the effects of the association of this type of ventilatory support and peripheral muscle activity, and to contribute to the intervention strategies being implemented as early as possible, aiming to improve the interaction of respiratory and musculoskeletal systems during exercise. In view of the larger scale use of NIV as an adjunct to the rehabilitation process, the objective is to improve the tolerance to exercise of COPD patients, reducing the expenses due to disabilities, hospitalizations and reducing the risk of morbidity and mortality in this part of the population.
