RESUMO:

Introduction: The natural history of Chronic Obstructive Pulmonary Disease (COPD) is associated with numerous systemic manifestations and comorbidities that complicate its evolution. Of these, dysfunction of the skeletal and respiratory muscles stands out as entailing a compromise in functional capacity. Even though muscle dysfunction is probably of multifactorial origin, there is evidence suggesting a link with systemic inflammation, demonstrated by plasma changes in systemic inflammatory mediators. Objective: The aim of this study is to investigate the correlation between systemic inflammatory mediators and strength of respiratory and peripheral muscles of upper and lower limbs in patients with COPD, as well as to compare values with healthy individuals. Methodology: this is an analytical and observational study; To be composed of 20 volunteers, of whom 10 were diagnosed with COPD (G1), aged 40 to 80 years; And 10 healthy and non-smokers, control group (G2), of the same age group. The collection will occur in a single day, where a blood sample will be collected to record the baseline values of inflammatory biomarkers - BI (IL2, IL6 and TNF-); Then the volunteer will be evaluated in the following sequence: pulmonary function test, palmar grip strength, manovacuometry, isokinetic exercise for lower limbs. For statistical analysis, the Shapiro-Wilk test will be used to verify the normality of the data; The descriptive analysis will be expressed by mean and standard deviation and median; The T-student test will be used to compare the variables studied in both groups; To analyse the correlation of BI levels with peripheral and respiratory muscle strength, as well as for comparison between groups, the Pearson correlation coefficient will be used in case of normal data distribution, or Spearman's correlation coefficient in case of Distribution of the data. For statistical significance will be considered p-value less or equal to 0.05. Expected results: We hope to better understand the behavior of systemic inflammatory mediators and its relationship with muscle strength in COPD, helping to improve the therapeutic management of these patients.
