RESUMO:

Background: Respiratory muscles respond to training just like as any muscle of the locomotor system. Muscular adaptations from inspiratory muscle training (IMT) follow the principles of physical training. Objective: To investigate the principles of intensity, specificity and reversibility of a IMT program in asthmatics. Methods: This is a double-blind randomized controlled trial. The participants were divided into two groups: group with training load of 15% of maximal inspiratory pressure (MIP) (TL15%) and group with training load of 50% of MIP (TL50%). Participants underwent assessments of lung function (included expiratory volume in 1 second (FEV1), forced vital capacity (FVC) and FEV1/FVC ratio), MIP, maximal expiratory pressure (MEP), functional capacity, quality of life related to health through the Asthma Quality of Life Questionnaire (AQLQ), asthma control by the Asthma Control Questionnaire (ACQ) and electrical activity of the inspiratory muscles (sternocleidomastoid and diaphragm) by surface electromyography (EMG). The IMT was performed with a frequency of 5 days per week for 6 weeks, consisting of 30 breaths twice a day. At the end of 6 weeks of training, participants were assessed. Results: To date were included 20 participants, of which three were excluded. Both groups significantly increased inspiratory muscle strength by about 18.00 cmH2O for TL15% group and 25.00 cmH2O for TL50%, with no difference between groups (p = 0.464). No effect of IMT on the MEP. Regarding lung function, there was no change in any of the parameters evaluated in the TL50% group, but there was deterioration in TL15% group for FEV1/FVC ratio (-0.04 ± 0.05, p = 0.039). Comparing the groups, the TL50% group showed better results for the FEV1% predicted (-4.11 ± 8.75 versus 3.83 ± 5.00%, p = 0.022) and the FEV1/FVC% predicted (-7.89 ± 6.23 vs. 0:02 ± 0:04%, p = 0.025) and absolute values (-0.04 ± 0.05 vs. 2:00 ± 4:51, p = 0.005). We also found an improvement of one (activity limitation) of the 4 domains the AQLQ in TL50% group after training (5.91 ± 0.90 to 0:58 ± 0:41, p = 0.022). The training was not able to improve the functional capacity or asthma control based on preliminary data.
