RESUMO:

Background: Stroke remains one of the most devastating neurological
conditions. A worldwide screening study of risk factors identified ten factors as
the most significant for the development of stroke. In parallel, practices of caring
for stroke survivors does not have a base on evidence, or a set of tools of easy
use that can identify long-term problems as well as facilitate guidelines to
primary care. The quality of clinical data can vary widely between low/middleincome
and high-income countries, thus, because of that the World Health
Organization (WHO) has recommended the STEPwise focus for stroke
surveillance. Objective: To identify the predictive variables of functional
independence in basic activities of daily living during hospital admission and at
the days 10 and 28 after stroke. Materials and Methods: This is a follow-up
study, carried out during one year in the Hospital Monsignor Walfredo Gurgel, in
Natal, State of Rio Grande do Norte. Participants were patients of both sexes
and any age, literate and illiterate, survivors of stroke, with neurological ability to
answer the instruments used in the study. It was excluded the severely-ill
patients, those in the Intensive Care Unit, those out of the institution for
examinations, those not possible to contact after hospital discharge, or that did
not accept to participate in the study. The study was approved by the Research
Ethics Committee. Two months before the start of the research, it was a pilot
study in the hospital with 21 patients. Twice a week, three examiners worked in
the identification of individuals admitted due to stroke. In the same day, the Step
1 instrument of WHO was applied. The data gathered that moment were
regarded to the hospital admission. The collection of days 10 and 28 was
conducted through phone contact in its majority. The data analysis was done
through the Statistical Package for Social Sciences software, being attributed a
level of significance of 5%. Results: The study included 433 stroke patients.
Most had suffered their first stroke, were literate, women, aged more than 60.
Ischemic stroke were the most common type found. One to three risk factors
for stroke were most present condition, as well as to be admitted in the hospital
less than 24 hours after stroke onset. After the logistic regression analysis, the
models for functional independence in basic activities of daily living identified in
its totality age, physical therapy care during in-hospital stay and situation of
patient at the day 28, as the predictive variables to independence. Other
variables such as number of risk factors and physical therapy care after hospital
discharge also were deemed in some predictive models.
