SUMMARY:

Illicit drugs and other chemicals consumed by humans are partially metabolized, eliminated in the urine and can be measured in sewage by LC-MS/MS after pre-concentration and using deuterated internal standards. Knowing the size of the population, the volume of wastewater and the excretion rate of the metabolite, it is possible to calculate the consumed amount of the drug based on its metabolites measured in the wastewater. This method, known as WastewaterBased Epidemiology – EBE, brings to light useful information for public health and safety and has direct repercussions on public campaigns for the prevention and mitigation of harm associated with drug addiction and environmental pollution. The objective of this project was to highlight some of the advantages and disadvantages of using the EBE tool in the diagnosis of cocaine and MDMA use in events in the city of Natal - Rio Grande do Norte. The quantitative method was calibrated using at least 5 curve points with a determination coefficient of up to 0.98. The results showed a consumption of 263 mg/1000 inhab/day for cocaine and 18 mg/1000inh/day for MDMA in Carnival and 3288 mg/1000inhab/day of cocaine and 36 mg/1000inh/day of MDMA in Carnival. In festive events consumption increases by approximately 2x and in the Christmas Carnival it was consumed around 12x than in Carnatal. The concomitant consumption of alcohol and cocaine was also evidenced by the presence of the metabolic substance Cocaethylene, a substance with greater toxicity than cocaine and ethanol and which must be a legitimate concern for public health. During the Christmas festivities and at the turn of the year 2021-22, the presence of mitragynine, Fentanyl and 25INBOH was detected in the wastewater. The cleanup study of dyes presents in sanitizers used in chemical toilets showed that SPE cartridges can be used to separate the dye from the analytes. The study provided relevant information for health and public safety managers, demonstrating the near real-time applicability of the technique, as well as highlighting limitations associated with sample preparation, but above all, it demonstrated the validity of the complementary use of the EBE tool in monitoring consumption of illicit substance
