Introduction - Congenital heart disease is the most common congenital defect and accounts for 8% of infant mortality, causing a major impact on morbidity and mortality in the pediatric age group. However, in Brazil, underreporting of cardiac malformations suggests that the diagnosis is made late, which prevents early surgical treatment. In Rio Grande do Norte (RN) the dimension of this situation is unknown, requiring greater detail, so that public policies to reduce morbidity and mortality due to cardiac malformations can be implemented at the state level. Given this scenario, the objective of this study was to describe the prevalence of surgical heart disease in children from the cases observed in the only referral center for pediatric heart surgery in the state, since its incorporation into a tertiary service in the capital of RN. Methodology – After approval by the Research Ethics Committee, a cross-sectional study was started with the analysis of electronic medical records of surgical cardiac patients admitted to the pediatric cardiology ICU in the state from October 2018, when the service was incorporated, to June 2021 The collected data were analyzed using descriptive statistics. The software used was JAMOVI 2.3.21. Considering the origin of the patients, the frequency of cases by municipality and Health Region was represented using maps. The software used was Qgis 3.22.6. Results – There were 393 admissions in the evaluated period, with an means age at admission of 2 years old, with 86% of the procedures performed by the SUS. It was observed that 41% of the patients resided in the 7th Health Region (RS), which includes the state capital and adjacent municipalities. When relating the number of surgical cases with the population of each region, a higher prevalence was observed in the Health Regions that are located in the territorial range to the south of the state. There was a predominance of procedures classified as RACHS1 severity score categories 2 and 3, with higher mortality in category 4 procedures. There were 43 deaths, with a mean age of 9.9 months, and cyanotic heart disease was detected in 79% of these cases. Conclusion – There is a high demand for pediatric patients in need of surgical procedures due to cardiac malformations in Rio Grande do Norte. However, it was found that the treatment is carried out late and concentrated in only one reference center, suggesting that it is necessary to implement public policies to carry out diagnosis and treatment in an earlier and decentralized way, reducing the morbidity and mortality of this disease. The higher prevalence of cardiac malformations observed in a certain territorial range of the state suggests that a deeper investigation is needed regarding the genetic and environmental factors involved in the etiology of heart diseases.
