SUMMARY:

Parkinson's disease (PD) is characterized by the death of pigmented neurons in the substantia nigra located in the midbrain and associated with Lewy bodies, which affects the central, peripheral and autonomic nervous systems, causing motor and non-motor deficits. Dysphagia is one of the most prevalent impairments in PD and the signs and symptoms can appear at any evolutionary stage, compromising the safety and efficiency of swallowing. The tongue performan essential role in the swallowing process and the change in tongue pressure can impair the preparation of the bolus in the oral phase and difficulty in transferring the bolus to the pharynx, which can cause aspiration and penetration in healthy individuals with increased risks in PD patients. Objective: To assess whether the efficiency and safety of swallowing is associated with reduced tongue pressure and its related factors in different stages of Parkinson's disease. Method: This is an observational, cross-sectional, descriptive study. The sample consisted of individuals with idiopathic Parkinson's disease (PD) and secondary parkinsonism with and without swallowing complaints. Data were collected at the Dysphagia for Parkinson's outpatient clinic at the University Hospital Onofre Lopes, where speech-language evaluations were performed, consisting of the application of protocols to verify the presence of signs and symptoms of alterations in swallowing, mastication and self-reported voice, evaluation of the maximum isometric pressure of tongue and swallowing test with food consistencies. Data analysis will cover descriptive and inferential statistics using the chi-square or Fisher's exact test, depending on the expected values of related variables and Student's and Pearson's t-test correlation tests, in addition to ANOVA for the factors associates, at a significance level of 0.05. Results: In article 1, it is assumed to analyze which evolutionary stage of PD that may be associated with the onset of dysphagia and which are the most frequent factors in PD that may be related, such as: severity of the clinical condition, polypharmacy, degree of dysphagia, functional performance, comorbidities and cognitive integrity, in which it will be possible to order the factors that can have a greater impact on dysphagia. Article 2 intends to evaluate the associations of swallowing efficiency and safety with the maximum pressure of the tongue, in the proposal to establish relationships between the peak parameters and the degree of oropharyngeal dysphagia and to be able to identify whether there are differences between patients with PD early-onset and late. The hypothesis of article 1 is that oropharyngeal dysphagia may be related to the factors of: polypharmacy, degree of dysphagia, functional performance, comorbidities and cognitive integrity, in which the factors that most impact the patient with PD will be ordered. The hypothesis of article 2 is that the evaluation of the maximum isometric pressure and the degree of dysphagia in Parkinson's will make it possible to identify the tongue pressure parameters that most frequently compromise the safety and efficiency of swallowing. The hypothesis of article 2 is that the assessment of maximum tongue pressure and swallowing performance will make it possible to identify the tongue pressure parameters that most frequently compromise the efficiency and safety of swallowing in patients with PD early-onset and late.
