TRANSIENT EVOKED OTOACOUSTIC EMISSIONS BY CONTRALATERAL SUPPRESSION IN BABIES WITH CONGENITAL SYPHILIS

SUMMARY:

Introduction: Syphilis is a global public health problem with complex challenges that present several efforts and demands for its control. In pregnant women, treponema paliidum can be transmitted to the fetus, causing congenital syphilis, being a risk indicator for hearing loss, as there is evidence of the presence of late sensorineural hearing loss related to it vertical transmission to the baby. Based on this premise, the efferent auditory pathway, in particular the medial olivocochlear region and the outer hair cells, can be assessed by Transient Evoked Otoacoustic Emissions by Suppression, contributing to the identification of the maturation of important physiological processes of this pathway in babies with congenital syphilis. Objective: Study 1 - Characterize transient otoacoustic emissions by suppression in children without risk indicators for hearing loss in the first year of life. Study 2 - Analyze the inhibitory effect of contralateral noise on transient evoked otoacoustic emissions in babies with congenital syphilis. Methodology: Study 1 - Systematic review registered in PROSPERO under number CRD42020187035, with searches in the Latin American and Caribbean Health Sciences (LILACS), PubMed, Scopus, Web of Science and LIVIVO The Search Portal for Life Sciences databases, in addition to research in gray literature and manual search in references of included studies. Study 2 - Cross-sectional and prospective, approved by the Research Ethics Committee nº 3.360.991. The total sample consisted of 33 babies, all of whom presented I-III-V waves in the Brainstem Auditory Evoked Potential at 80 dB HL and wave V at 30 dB HL, presence of bilateral non-linear transient evoked otoacoustic emissions (TEOAE). The suppression analysis was performed using linear TEOAE without noise at 60 dB SPL and with contralateral white noise at 60 dB SPL. The mean of inhibition was obtained by subtracting the TEOAE without noise from the TEOAE with noise. Partial results: Study 1 – 2688 articles were found, after the selection steps, 15 studies met the eligibility criteria. The publication period varied from 1994 to 2019, the stimuli used were: linear, non-linear and the unfiltered click. Regarding age, studies present data up to 6 months, with a gap up to 12 months of age. All articles demonstrate TEOAE suppression in subjects without risk indicators with an average varying between samples. Study 2 - the Study Group (SG) composed of 20 babies with congenital syphilis, 12 (60%) male and 8 (40%) female and the Control Group (CG) formed by 13 babies without risk indicators for loss hearing, being 5 (38.46%) male and 8 (61.5%) female. When analyzing the TEOAE amplitudes without noise and with noise, smaller amplitudes were observed for the study group in both conditions, when analyzed according to sex, the amplitudes were greater for women in the study group and in men for the control group bilaterally. Preliminary conclusion: Study 1 - Efferent inhibition occurs in babies without risk factors for hearing loss in protocols with different capture characteristics, generating average responses that do not allow a normative clinical value to date. Study 2 - The inhibitory effect of contralateral noise on TEOAEs of babies with congenital syphilis does not differ from those without risk factors for hearing loss, with qualitatively lower amplitudes for each test condition.
