SUMMARY:

Introduction: Effective communication was established by the World Health Organization as an international goal of Patient Safety, aiming to promote more qualified and safer care practices in hospital institutions. Failures in communication processes between health professionals are related to care errors that result in patient harm, and occur mainly in care transitions. Guidelines established by the Joint Commission International suggest that the use of technological tools promotes improvements in the transfer of critical information during care transitions in the hospital context. Objective: To investigate the effectiveness of the use of medical informatics applications in communication between health professionals during the care transition processes of hospitalized patients. Methodology: This is a systematic review, based on PRISMA method guidelines, divided into 4 phases: Research protocol, analysis, synthesis and publication of results. The guiding question of the research was elaborated according to the components of the acronym PICO. The search terms were defined according to each acronym, where P “Patient Care Team”, I “Medical Informatics Applications”, C “Medical Records” and O “Patient Harm”. Consultations were carried out in PubMed, Embase, LILACS, Cinahl and in the gray literature. The following inclusion criteria were established: observational studies and clinical trials that report the use of any technological tool used in communication between health professionals in any care transition process in a hospital context. Results: A systematic review protocol was prepared and submitted to the PROSPERO platform. The search strategy will be performed in the databases and will be exported to Rayyan for blind evaluation by revisors R1 and R2, who will read the title and abstract. Data will be extracted later for analysis and risk of bias, including meta-analysis, if possible. Conclusion: It is expected to obtain evidence on the contribution of the use of technological tools to improve communication between health professionals in care transitions in the hospital context with a strong degree of recommendation. From the systematic review carried out, a technical report will be prepared to be made available to the Ebserh Network and other hospitals in order to contribute to the improvement of communication between health teams, with positive consequences for patient care.
