Syphilis is a systemic, chronic, curable bacterial infection exclusive to humans. Despite advances in prevention, diagnosis and treatment, syphilis is considered a re-emerging disease in Brazil, as well as in other countries, remaining a serious public health problem. The general objective of the study is to analyze the reported cases of syphilis in pregnant and congenital women in the Seridó region of Rio Grande do Norte in the period from 2011 to 2021. This is a research with a quantitative epidemiological approach, of the descriptive observational type, in which Secondary data on confirmed cases of syphilis in pregnant women and congenital syphilis from the SINAM and SINASC databases were used. The studied population was made up of all reported cases of syphilis in pregnant women and congenital syphilis, from January 1, 2011 to December 31, 2021, in the 4th health region of Rio Grande do Norte. The data were presented in graphs, tables and expressed in absolute and relative frequencies. This research is in accordance with the Guidelines and Regulatory Norms for Research involving human beings, Resolution 466/12 of the National Health Council, and submitted to evaluation by the Research Ethics Committee of Facisa/UFRN (CAAE: 64899922.2.0000.5568) and the substantiated opinion number 5,826,478, with reporting date of 12/19/2022, reports the approval of the research without any pending issues or inadequacies. Of the 305 reported cases of syphilis in pregnant women, 116 had vertical transmission and of these cases in which there was vertical transmission, 38.7% of mothers were diagnosed with syphilis at the time of birth, demonstrating a failure in diagnosis and treatment in Primary Health Care, and 94.8% received prenatal care. We observed that only 8.6% of mothers with VT underwent adequate treatment for maternal syphilis up to 30 days before delivery, leaving 91.4% with inadequate treatment, not carried out or ignored. Taking into account that pregnant women must be tested for syphilis, at least, in the first prenatal consultation, at the beginning of the third trimester and during hospitalization for childbirth, this data demonstrates that the active search in the first trimester of pregnancy is flawed, being around 60% of cases diagnosed from the 2nd trimester of pregnancy. According to the data collected, it can be concluded that the reported cases of syphilis in pregnant women and congenital syphilis represent a serious public health problem in the municipalities of the 4th health region of Rio Grande do Norte, and can also relate cases of congenital syphilis to quality of prenatal care in Primary Health Care and the difficulty in understanding the notification criteria for cases of congenital syphilis.
